01 Our mission

Serious clinical care, at human scale.

A Congregate Living Health Facility (CLHF) is a licensed residential healthcare facility for patients whose needs are too complex for ordinary residential care, but who generally no longer require an acute-care hospital. Care is more medically intensive than a typical skilled nursing setting — 24-hour skilled nursing, ventilator support and management, tracheostomy care, complex wounds, IV therapy — delivered in a six-bed residence where every patient is known by name, by history, and by what matters to them.

A CLHF is not assisted living, a board-and-care home, a nursing home, a hospice residence, or a private home with caregivers. It is a regulated healthcare facility where medically complex residents receive continuing nursing and clinical support in a residential setting.

Our mission: to provide compassionate, clinically responsible, and individualized care for medically complex individuals in a safe, dignified, and home-like residential environment — combining the clinical discipline of a licensed healthcare facility with the personal attention a six-bed residence makes possible.

02 Why Calmora

What we're building differently

i.

A home, developed for complex care

Each property is being developed and equipped specifically for this level of care: natural daylight, quiet rooms, and a garden outside a real window. Environment shapes recovery — so we treat the home itself as part of the plan of care.

ii.

A partner hospitals can count on

A structured clinical review of every referral, honest answers on capability and availability, and coordinated transition planning. Discharge should be a warm handoff, not a leap of faith.

iii.

Families stay part of the team

Open visiting, plain-language updates, and a care team that explains every decision. Healing goes better when the people who love you are in the room.

03 In development

Three residences, purpose-developed for complex care

Each CALMORA residence is being developed as a licensed six-bed home — residential in scale and feel, with the clinical infrastructure medically complex care requires.

04 Clinical programs

Planned clinical capabilities

Respiratory & ventilator care

Ventilator support and management, tracheostomy care and suctioning, airway management, oxygen therapy, and respiratory monitoring — in coordination with respiratory therapists, pulmonologists, and equipment providers.

Complex skilled nursing

24-hour skilled nursing: assessments and monitoring, medication administration and management, complex wound care, catheter and ostomy care — with IV therapy and line care based on physician orders and facility capability.

Rehabilitation therapies

Physical, occupational, and speech therapy — provided or coordinated according to each patient's individualized plan of care. Progress is measured in what matters: a first step, a first word, a meal at the table.

Whole-person support

Enteral nutrition and dietary support, social services, recreational programming, family education and care conferences — and discharge and transition planning from the day of admission.

Who we plan to care for

Proposed patient profile. Every patient undergoes an individual clinical review before admission — the ability to admit depends on licensure, physician orders, staffing, equipment, and bed availability.

05 Our philosophy

Care designed around people

CALMORA's leadership brings experience across healthcare operations, healthcare-facility development, regulatory and licensing coordination, post-acute and long-term care, hospice and palliative care, and complex wound-care operations. The standard is simple: every protocol clinically responsible, every room filled with daylight, every family greeted by someone who knows their name.

“We are building CALMORA because medically complex patients deserve more than simply a place to stay after leaving the hospital. Our goal is to create smaller residences where skilled care, dignity, family involvement, safety, and personal attention are incorporated into every patient's plan of care.”

07 Admissions

How a referral becomes an admission

Every referral follows the same structured path — so hospitals, families, and health plans always know where things stand.

  1. 01

    Referral or inquiry

A hospital, case manager, physician, health plan, family member, or authorized representative contacts Calmora.

  2. 02

    Clinical review

Records are submitted securely. A qualified clinical professional reviews medical condition, nursing and respiratory needs, medications, nutrition, equipment, rehabilitation needs, and safety risks.

  3. 03

    Capability assessment

We confirm whether the residence has the license, bed availability, staffing, competencies, and equipment to care for the patient safely.

  4. 04

    Payer verification

Eligibility, authorization, benefits, and payer requirements are reviewed.

  5. 05

    Transition coordination

On acceptance, we coordinate physician orders, pharmacy, respiratory and durable medical equipment, nutrition, transport, and the discharge plan.

  6. 06

    Admission & plan of care

The nursing and interdisciplinary team completes required assessments and begins the individualized plan of care.

Planned payer sources

All payer participation is subject to enrollment, contracting, authorization, and applicable benefits.

08 For hospitals & case managers

Start a referral conversation

We welcome pre-opening referral inquiries. We aim to acknowledge inquiries within one business day; the time required for a complete clinical review depends on the availability and completeness of medical records, patient complexity, bed availability, staffing, equipment requirements, and payer authorization.

  • Structured clinical review of every referral
  • Direct coordination with discharge planners and case managers
  • Honest answers on capability, availability, and payer authorization

Please don't include protected health information.

Submission of a referral does not guarantee admission. Every referral is subject to clinical review, licensing requirements, bed availability, staffing capability, equipment availability, and payer authorization.

Patients & families

What to expect

Admissions guidance, insurance help, open visiting, and honest answers at every step. Ahead of opening, we can help you understand whether a CLHF is the right level of care for your family — no obligation, no jargon.
